About the Role
Peaceful Hearts Hospice is seeking a compassionate, experienced Registered Nurse Case Manager (RNCM) to provide skilled, end-of-life nursing care to our patients across all care settings. In this role, you'll take ownership of a patient/family caseload — assessing needs, developing plans of care, and coordinating with physicians and our interdisciplinary team to ensure every patient receives dignified, high-quality hospice care.

Reports To
Director of Patient Care Services (DPCS)
What You'll Do
Own a caseload (averaging 12 patients) — assessing, planning, implementing, and evaluating each patient's nursing care
Coordinate with attending physicians, the interdisciplinary team (IDT), and other agencies to ensure optimal patient care and resource use
Assess and reassess patient needs on an ongoing basis, adjusting the plan of care as conditions change
Provide skilled nursing services including wound care, symptom management, tube feeding/care, and bowel regimen management
Educate patients and families on disease progression, end-of-life symptoms, and what to expect
Supervise and support LPNs and Hospice Aides, including documented care observations and competency evaluations
Participate in a shared on-call rotation providing 24/7 coverage as needed
Maintain timely, accurate documentation in our electronic medical record system (charting completed within 24–48 hours)
Provide emotional support to patients, families, and caregivers — including at time of death, with time-of-death pronouncement per policy
Manage admissions, discharges, transfers, and revocations, including reviewing referral records and coordinating transitions of care
Attend and actively participate in IDT meetings, stand-up/stand-down meetings, and all-staff meetings
Uphold the mission, vision, and philosophy of Peaceful Hearts Hospice in all interactions
What You'll Need
Education & Licensure
Current, unencumbered Registered Nurse license in the state of Nebraska
Graduate of an accredited school of Nursing
Experience
2+ years of RN experience preferred
2+ years of experience in hospice, home health, or oncology preferred
Other Requirements
Valid driver's license, auto liability insurance, and reliable transportation
Annual TB test and other state-required health screenings
Ability to travel between patient homes, facilities, and the office as needed
Work Environment & Physical Demands
This role is primarily field-based, with time in the office for meetings, charting, and calls. You'll need to be comfortable with:
Regular kneeling, squatting, reaching, twisting, climbing, and walking
Occasional lifting/transferring of patients
Exposure to varying temperatures and humidity in different care settings
Sitting (2–4 hrs/day), standing (3–5 hrs/day), and walking (2–4 hrs/day)
Frequent phone and computer use
Working under pressure in a fast-paced, emotionally demanding environment
Occasional shifts longer than 8 hours
You should also bring strong time-management skills, sound independent judgment, emotional resilience, and the ability to work compassionately with people from all backgrounds and belief systems.
