In this role, you'll work in the field completing member assessments for those individuals residing in Nursing Facilities who wish to relocate to a community setting of their choice.
You will also conduct new referral assessments for those members wanting to upgrade to waiver services and for those individuals who are living in the community and needing waiver services. At every turn, you'll have the support of an elite and dynamic team. If you are located in or within commutable driving distance from Pasadena, Baytown, or Houston, Texas, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
- Visit Medicaid members in their homes and / or other settings, including Nursing Facilities, Community Centers, Hospitals or Providers' Offices
- Provide a complete continuum of quality care through close communication with members via in-person, telehealth or on-phone interaction
- Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels and Minor Home Modifications to name a few

Required Qualifications:
- Current, unrestricted RN license in the State
- Valid driver's license, reliable transportation to travel up to 75% to member homes or other locations within service delivery area
- 2+ years clinical experience in a hospital, acute care, home health / hospice, direct care or case management
- Intermediate proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
- Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
- Proven ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from other
- Reside in or within commutable driving distance from Pasadena or the South Houston, TX area
Preferred Qualification:
- Case Management experience including Certification in Case Management
- Medicaid, Medicare, Managed Care experience
- Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
- Experience in Home & Community based or Long Term Care services delivery
- Home care / field based case management
- Bilingual skills: English and Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
